RUVI Health PLLC offers secure, HIPAA-compliant telehealth services. You may opt out of electronic communication at any time.

With your consent, we may send SMS messages for appointment confirmations, care instructions, and administrative updates. You may opt out anytime by replying STOP. Standard message rates may apply.

Disclosures Requiring Authorization

We will obtain your written authorization before using your PHI for marketing purposes or sale of PHI.

Your Rights

You have the right to:

  • Revoke this consent in writing at any time and all full disclosures will then cease.

  • Access and obtain copies of your medical records

  • Request corrections

  • Receive an accounting of disclosures

  • Request restrictions on certain uses
